Vitalis are recruiting for an experienced Social Worker to work in an Adult Community Team in the Wirral.
An experienced adult social worker is required to undertake Care Act assessments and reviews, CHC checklists, MCA etc. Services to be reviewed, range from Domiciliary care, Supported Living, 24 hour placements, Extra Care Housing, Day services etc The candidates are required to have sound knowledge of the Care Act 2014, Mental Capacity Act 2005. Must be able to undertake MCA’s and best interest assessments. Have sound knowledge, experience and skills in respect to safeguarding. Ideal candidates must have experience in regard to Continuing Health Care and have attended CHC/MDT’s. Ideal candidates must be prepared to undertake face to face visits.
