Pay rate £30.51 per hour
Vitalis are recruiting for an experienced Social Worker in to work in a Mental Health Team in Sutton.
This role is for a Mental Health Social Worker working within the local authority mental health social care team, specifically working on hospital discharges from mental health hospitals. The Social Worker should be experienced in managing complex Adults Mental Health cases and completing statutory Care Act tasks, specifically Care Act assessments, reviews and care planning. They should have experience working alongside multi-disciplinary professionals, S117, attending ward rounds, working with Housing and brokerage, Court of Protection applications, Mental Health Tribunals, carrying out Safeguarding Enquiries and duty work.
