Vitalis are recruiting within for a Qualified Social Worker to work in a Transforming Care Team in Leicestershire County Council. 
Transforming Care social worker post is a post that is specifically for Hospital discharge for complex Learning Disability and/or Autism and Mental Health patients
I will need someone with experience in completing tribunal/court paperwork and ideally some hospital/section 117 experience
