Contract role – Leaving Care 16+ Team 
Vitalis are recruiting for an experienced Personal Advisor in Buckinghamshire 
This is to work in a Leaving Care 16+ Team in Aylesbury. 
You will need to have experience of Pathway Planning and access to your own transport.
