Vitalis are recruiting for an experienced Social Worker to work in a Mental Health and Learning Disability Team in Somerset Council.
To undertake assessments of risk and care needsin relation to service usersand carers so that, within relevant legislation, appropriate action is taken, monitored and reviewed effectively.
