Vitalis are recruiting for experienced Social Worker to work in a Preparation for Adulthood Team in Warrington Council.
Experience working within a transition team essential. 
Experience working with Learning Disabilities, Autism preferred 
Hybrid working – in office 2-3 days a week essential
